Gastro-intestinal cancer is a vast topic that covers cancers of esophagus to anal canal. Inbetween we have cancers of stomach,small intestine,colon,rectum and anus. It also includes cancers of solid organs like cancer of liver,bileducts and pancreas.

Most of these cancers are Adenocarcinoma – meaning glandular cancers, where in surgery is the primary modality of treatment. Radiotherapy maybe helpful in some anal canal cancers.

Diagnosis is usually done by simple tests like – Upper GI endoscopy, colonoscopy, USG,or Ct Scan.

GI cancers follow a particular pattern usually based on the blood supply and lymphatic drainage of the particular organ.

Surgical removal is usually followed by good periods of control of the disease,and it involves Radical resections for good disease control. This is especially true in cancers of colon and pancreas. Similarly the survival rates are poor in cancers of stomach and esophagus

I have done about 250 surgeries- esophagogastrectomy-for esophageal cancers with 5 yr survival of about 35%. Small intestinal cancers are rare and I have done about 10 cases- A special case was Cancer of the Ileostomy done for IBD about 35 yrs back. Removal of the cancer with reconstruction was done.-only 36 cases reported in the history.

I have operated on many -1200- pts with colorectal and anal cancers with good survival, except for Stage 4 disease where the survival is poor

Cancer of the liver and Gall bladder is not my domain and I have done only a few cases with poor survival.

For good survivals we require a team involving Medical and Radiation Oncologists.

Surgery can be done by Open,Laparoscopic or Robotic approach based on the patient. Robotic surgery has changed the way we do surgery with better results and increased safety to patients.
